In order to ensure compliance with best practice guidelines we require a service level agreement for our CBCT scanning service. Please download, complete and return the service level agreement in order to start referring your patients for CBCT scans.

Your completed and signed service level agreement can be returned to us scanned in via email to b.dixon@redmiresdentalcare.co.uk or by post.

We will contact the patient directly to arrange an appointment. Patients will be cared for only as requested and will be returned for the definitive restoration. Upon completion of treatment a clinical report will be provided for the patients records.

For patients where urgent management is required, no referral is necessary; please phone us for an express appointment.
